We are currently accepting new & return individual clients, Couples/Marriage (self-pay only), Mental Health and S.A.P. D.O.T. Evaluations, however, depending on your needs determined by your Client Intake Form you could be placed on a short waiting list.

If you are filling out this form for an individual that is <14 years old, you MUST be the legal Parent/Guardian and fill out this form with the child's name. If the Appointment is being requested for an individual 18+ years old, this Request Form MUST be filled out by the individual needing the appointment, NO EXCEPTIONS! All Fields are Required Fields.
