We are currently accepting new & return individual clients, Couples/Marriage (self-pay only), Mental Health and S.A.P. D.O.T. Evaluations, however, depending on your needs determined by your Client Intake Form you could be placed on a short waiting list.

If you are filling out this form for another person that is 14 years old or older, you must fill out the form with their name, phone number and email address. We do not accept any Medicaid Products. All Fields with a Red * are Required Fields.
